Provided is a light source device that is capable of combining and emitting a plurality of wavelengths of laser light, has a simple structure, and enables a decrease in device size.　The light source device comprises: a stem; a plurality of laser diodes fixed to the stem and having different emission wavelengths; a heatsink disposed on a main surface of the stem; a light guide member disposed on a main surface of the heatsink that is non-parallel to the main surface of the stem; a cap covering an outer periphery of a region away from the main surface of the stem with respect to a first direction parallel to the first main surface of the light guide member; and a light extraction window provided in a part of the cap. The light guide member includes: a grating coupler into which light from the plurality of laser diodes enters and which converts the direction of travel of the light into the first direction; and a planar waveguide that guides the light entering via the grating coupler to a first end portion on the side closer to the light extraction window of the light guide member, and that is integrated with the grating coupler.
